As Certified Community Behavioral Health Centers (CCBHCs) continue to populate the country, care coordination, data analytics and evidence-based practices are becoming the new norm for human services organizations. CCHBCs empower organizations to provide more effective care at a lower cost. In order to succeed and serve in a value-based care model, Care Plus New Jersey (Care Plus NJ) took the proper steps and facilitated new workflows in order to excel as a CCBHC.

Transforming Care: the CCBHC way

CCBHCs provide an integrated model of care that represents a significant step forward in improving outcomes, aligning provider payment with outcomes and recovery while decreasing the cost of care. They are required to integrate behavioral health, physical health and social services organizations into their care model and programs — while also providing care coordination to manage all these services for people they serve, many of whom have multiple co-occurring mental health, substance use and physical health disorders.
