As Certified Community Behavioral Health Centers (CCBHCs) continue to populate the country, care coordination, data analytics and evidence-based practices are becoming the new norm for human services organizations. CCHBCs empower organizations to provide more effective care at a lower cost. In order to succeed and serve in a value-based care model, Care Plus New Jersey (Care Plus NJ) took the proper steps and facilitated new workflows in order to excel as a CCBHC.

CCBHCs provide an integrated model of care that represents a significant step forward in improving outcomes, aligning provider payment with outcomes and recovery while decreasing the cost of care.
